| Details: | SUMMARY: Called regarding failure to submit public notice and pending civil penalties DETAILS: I called John Staver to inform him that the Program was intending to assess a $500 penalty due to the fact that the Program had not received a copy of the tier one public notice and verification that it had been distributed. I reminded John of the letter issued by the Program on October 20 and his voicemail message from October 24 confirming that he had received the letter and was distributing the notice as required. John states that he hand delivered the public notice on October 25 and had residents sign to verify that they received the notice. He stated that he did mail the notice and verification back to the Program, but it has not been received to date. John then stated that he would immediately re-send the notice and verification to the Program. ACTION NEEDED: None | ||
