Details:	SUMMARY: Source Water Assessment (SWA) samples DETAILS: New operator confuse on routine total coliform sampling and required completion of SWA samples. I called the state to request direction on how operator can complete SWA since February and April sampling was missed. 6/24/12 -Attempted contact with operator. Left message on required action followed up with mailed copy. ACTION NEEDED: Per Evan Hofeld: Complete SWA by sampling at the well tap in July and November of 2012, then in February and April 2012. The February 2012 sample will also satisfy the systems ground water rule annual assessment. Operator understands that quarterly total coliform sampling in the distribution system is an ongoing routine sample that is also required. Next routine total coliform sample due second quarter by the end of June.
