Details: SUMMARY: Priority non-Complier (PNC) DETAILS: 1/11/12 Contacted Ramona and left a message about the positive coliform and discussed next steps. 1/18/12 Repeat samples taken 2 weeks late. 2/29/12 Site visit made for survey. I went over all aspects of coliform sampling plan with operator. I also informed her they were required to do monthly source water sampling. 3/2/12 System has another coliform positive sample. I called Ramona to remind her to take the repeats immediately. I also asked about the chlorine residual because state isnt showing a residual when the sample was drawn. Repeats were never drawn after this positive. 3/22/12 Ramona called to find out how many samples she needed to take this month. I told her to take the 5 temporary routines in distribution. She said they were having problem with the pump on the chlorinator. They had Stutzman out to work on the chlorinator earlier in the week. I reminded her they needed to take a source sample. 3/29/12 System drew 4 routines. One of the samples was positive. 4/2/12 Contacted Ramona and told her to take the repeats immediately. I also told her she needed to be documenting the residual. I called Stutzman and said they were still having problems. 4/16/12-5/20 Tried numerous times to get a hold of Ramona and she hasnt returned any calls. 5/31/12 Called the owner James. He said he old follow up with his lab about sampling. 6/6/12 Met with owner on site to go over deficiencies from survey. He said he just took the source sample and the alb was coming to pick it up. System sampled the source on 6/7 and reported on 6/14. All source violations were returned to compliance. on 6/14/12. System score is now one. ACTION NEEDED: none
