| Details: | SUMMARY: System survey deficiency follow up DETAILS: Water system survey deficiencies included the following: Well head not protected from flooding. Well does not meet setback from hazard. Incomplete O and M manual. Improper spigot installation. Concrete slab fracture. Poor drainage around wellhead. A valid phone number was not found for this system. A letter was mailed on 7/3/12 describing needed actions to verify deficiency resolutions. 7/41/12 Visited the site and confirmed most of the deficiencies were corrected. The ditch near the wellhead was filled in, the spigot was fixed and the concrete slab was fixed. They still need to put together an O and M manual and send us a copy. Dee in 2 weeks. 8/9/12 Very brief O and M manual received. ACTION NEEDED: none | ||
