Details:	SUMMARY: Violation of Notice of Compliance for failure to take Nitrate sample and total Coliform samples by 12/31/11 DETAILS: Violations 802724813, 902724816, 902724815, 902724814, 902724812, 902724811, 902424810, 2724814, 2724813 and 2724812. Discussion that he was in violation of drinking water rules had not complied with notice was not only at risk of civil penalty assessment but of license revocation for failure to be in compliance with drinking water rules. Mr Brown stated that he will take the samples next week and turn them in to Delta Labs in Eugene. 5/1/12 Spoke to on-site manager Vince Elliott, conveying results of our coliform sample, negative, and that the coliform test he took was not reported into SDWIS. Also told him that all they needed to do now to be on their way back to compliance was take a nitrate sample and instruct the lab to forward to the state. He has no e-mail. ACTION NEEDED: Take a Nitrate and Coliform sample and submit results to State DWS, as well as fax copies to Douglas Co Env Health.
