Details:	SUMMARY: Follow up to system survey significant deficiency items DETAILS: I have spoken with Doug a few times since the survey but I called him on 10/3/12 to let him know I received the annual CCR report and had faxed it to the state. The state received this dated 10/2/12 in website. He also mentioned to me he has switched over to Clorox bleach for use with the chlorinator. These two items have been corrected as noted from the system survey and all items as identified in the survey are now corrected. ACTION NEEDED: none
