Details:	SUMMARY: Deficiency survey follow up DETAILS: Survey date 3/20/13 . Sent letter to operator stating the following: Well does not meet setbacks from hazards. A waiver was submitted and conditional approval was given. To maintain the approval you needed to work with your diesel fuel supplier to ensure spill prevention was in place during filling. You worked with the fuel supplier on the spill prevention procedures. These procedures will be reviewed at future surveys. This deficiency has been corrected. Well head is not protected from flooding. You provided a functional sump pump. This deficiency has been corrected. No Emergency Response Plan or Operation and maintenance manual. You provided me with complete copies of both of these documents of February 6 2013 These deficiencies have been corrected. ACTION NEEDED: All deficiencies have been corrected.
