Details:	SUMMARY: 2011 & 2012 Nitrate monitoring violation 2012 4th quarter routine TC violation DETAILS: 6/19/2012 contact with Joe Tavera regarding annual nitrate monitoring. Confirmed that 2011 sample was taken and correct lab documentation in county file, in addition 2012 sample done Feb 2012. Joe to fax 2012 lab results to county office and I will forward to the state on the systems behalf. Copy of 2011 and 2012 labs to be scanned to DMCE 6/18/2012.10/18/2012 Contact from Oregon DWP, the DEQ lab ODOT is using is not accredited for doing potable water samples for drinking water systems. Both the 2011 & 2012 are invalid. State to notify the system of this issue.11/5/2102 Received email from Jose confirming ESC took samples for nitrates.Copy of my email correspondence with Jose regarding number of samples and how to submit to state: Thanks Jose for letting me know the sample was taken. Will ESC electronically send the results directly to the Oregon Drinking Water program for the system? Some labs do others do not. If not, either I can forward for you or you can send directly yourself. Just let me know if you need assist once the test results are back. See file. ACTION NEEDED: 6/19/2012 No follow up needed. System back into compliance on nitrate monitoring.10/15/2012 Remailed lab results to DMCE as not showing in SDWIS yet.2/5/2013 Joe to check with admin support to make sure lab forms get submitted to DWP. Plan to scan and emailed to DMCE 2012 ESC nitrate lab results and 4th quarter TC. THIS WAS NOT DONE BY MAUREEN TAYLOR I noticed that the lab form for Source Assessment faxed to me was not the correct year- it had a 2010 date on it, plus the ESC lab form was not the nitrate results. 2/8/2013 message left on ODOT phone.
