Details: SUMMARY: Deficiency follow up DETAILS: The information on the well that you have provided doesn’t fit with the observations I’ve made when on site. I’ve attached pictures of the well(s) from 2008 and 2013. I’m wondering if maybe the well in the picture from 2013 is possibly a new well – not the same well as pictured in 2008? Linn County Environmental Health (my department) did not receive any plans regarding the well. Linn County Planning and Building did receive plans for the new building next to the well. I was able to get the building official to pull the plans and look at them with me yesterday. Page C4.0 shows an aerial view of the well house but not an elevation profile. It shows an increase in elevation of less that one foot. Planning and Building does not have jurisdiction over wells. Wells that are located in the flood plain must have the casing extend at least 2 feet above the anticipated 100 year flood plain. That’s why the well pictured in 2008 was so tall. This deficiency still needs to be corrected. To do this, you will need to go through plan review. Please contact Jay MacPherson for plan review requirements.2) This deficiency has been corrected.3) You need to be submitting the “Monthly Disinfection Report for Ground Water Systems” (attached) monthly to: DMCE, Drinking Water ServicesP.O. Box 14350Portland, OR 97293=0350 This report must be received on or before the 10th day of the following month. 4) How long do you think you will need to get the information?I would be happy to meet with you on site. Please let me know if you would like to set up a time to do so.Are you still the contact for the water system? I thought the new contact was Jeff Roy. Please let me know if I need to change the contact information back to you." ACTION NEEDED: Deadline for correction is 10/9/13
