Details:	SUMMARY: Survey deficiency follow up re plan review components DETAILS: Telephone contact with Eric Chamberlain regarding water quality complaint.Eric confirmed that 2012 survey deficiencies have been completed.- Pictures of NSF approved chlorinator documented while I was on site last week. See attached-Well head has been refinished. Company that did the refinish work stated the well head seal did not need to be replaced.11/15/2013 email sent to Evan Hofeld via DMCE with pictures of chlorinator. ACTION NEEDED: -12/4/2013 contact sheet and pictures emailed to Evan H. to confirm that he received them from LPHJ.
