Details:	SUMMARY: MCL- repeat total coliform positive for sample date 6/23/14 DETAILS: I contacted to the operator to let him know that the spring sample & all distribution samples were total coliform positive. The operator feels that he knows why the system has total coliform. He found a slug in the spring box. The lid is not adequately sealing to the spring box. He will repair the seal to provide an adequate seal to prevent entry of unwanted source of contamination. He will disinfect the spring & distribution system. I let him know that a public notice would be needed & emailed him the public notice template. I let him know to take 5 temp routine samples in July. ACTION NEEDED: Monitor follow ups & respond as necessary. Obtain completed public notice.
