Details:	SUMMARY: Survey Follow Up DETAILS: It was identified that the wells at this facility are with in 100 feet of the onsite septic system drain field. This system has always been this way since the first survey in 199. Each survey since this time has reference that the drain field is with in 100 feet of the well heads. It was made clear by the State that this now needed to be marked as a deficiency and have Mobilife address the issue. Mr. Gilham has sent a letter in response to the deficiency notice in the survey stating that there corrective action plan is to continue to monitor the drinking water and when it no longer meets the Oregon Drinking Water Programs bacteria requirements or 75 years in the future, at which time they will connect to EWEB or Willamette Water Company to supply drinking water to the subdivision. ACTION NEEDED: None, all violations have been corrected. A copy of the Action Plan is begin submitted with this contact report and will be referenced at each sanitary survey.
