Details: SUMMARY: WSS Deficiencyfollowup DETAILS: The system has not submitted a Cross Connection Enabling Authority to the Oregon Health Authority, Drinking Water Division (OHA). I have left paperwork with you to assist the water system to complete the necessary paperwork. The Cross Connection Program requires a system to have a list of all devices and have them tested on a yearly basis. Please provide documentation of the testing of the devices. completed June 1, 2015The Cross Connection Program requires a system to submit an Annual Summary report. I left a copy of the 2013 Summary report with you at the time of the survey. If you need any further assistance with the Cross Connection Program the contact at OHA is Michael Perry (971) 673-1220. The system is required to have a current written Coliform Sampling Plan. A copy of the document was The system is has not published a Consumer Confidence Report yearly. All community water systems are required to submit an annual Consumer Confidence Report (CCR) to their customers. A CCR must cover the previous calendar year, and be delivered to customers and a copy sent to the OHA Drinking Water program by July 1st. The system must also certify the CCR and send the certification form to OHA DWP by October 1st.The system is missing a Public Notice from December 2012 required due to the confirmed positive Total Coliform. Please submit a copy of the Public Notice distributed to your customers.State no longer is requiring system to comply as of Feb 28, 2011The system does not have an Emergency Response Plan (ERP). When the ERP is complete, the form in the back of the notebook needs to be submitted to OHA and I need to be notified as well. ACTION NEEDED: The system has completed all significant deficiencies from WSS
