| Details: | SUMMARY: Deficiency follow up DETAILS: for water survey conducted on July 13th 2015.The significant deficiencies and rule violations noted are as follows:1. OAR 333-061-0036(6)(b)(G) A coliform sampling plan is required that lays out when and where routine and follow-up sampling will occur. Ideally you would designate three representative rotating sample sites for regular quarterly sampling. Systems that chlorinate must also submit an annual raw water sample from the well. 2. OAR 333-061-0065(4) You have not yet drafted an Operations and Maintenance Manual for this system. 3. OAR 333-061-0064(1) This new system does not yet have an emergency response plan ACTION NEEDED: Part of the deficiencies have been corrected. Operator is not available at this time. Deficiencies should be corrected before 12/31/2015. | ||
