Details:	SUMMARY: Regulatory Assistance Regulatory Assistance DETAILS: A sanitary survey was completed on August 5, 2015 as part of reactivating the store as a PWS. The system had multiple significant deficiencies. The following deficiencies have not been corrected 1) install well vent, 2) protect well from flooding, 3) coliform sampling plan, 4) emergency response plan, 5) Operations and Maintenance Manual.A follow up visit was made to the system on December 18, 2015. Significant deficiencies 1 and 2 were not corrected. Significant deficiencies 3, 4, and 5 could not be evaluated because the owner was not present. An enforcement letter was mailed on December 21, 2015 and set a deadline ofJanuary 31, 2016 to complete corrective action ACTION NEEDED: Correct remaining significant deficiencies by January 31, 2016.
