Details:	SUMMARY: Deficiency Follow up for Water Survey Conducted on September 30, 2015 DETAILS: Significant deficiencies and rule violations are noted.Corrections are still needed as follows:Extension is needed for the operator to make all of the corrections. Operator has been given until April 6th to finish all corrections. Operator is attempting to make all of the corrections by April 6th 2016. ACTION NEEDED: Continue working on corrections for the Porter Creek Mobile Home Park and correcting all deficiencies by April 6th 2016.
