| Details: | SUMMARY: Regulatory Assistance DETAILS: A sanitary survey was completed on August 5, 2015 as part of reactivating the store as a PWS. The system had multiple significant deficiencies. The firm did not collect all deficiencies. An enforcement letter was drafted on March 31, 2016 by DWS and mailed to the system. I was contacted via phone by Jim Edwards and informed all deficiencies were corrected. I visited the system on April15, 2016. All deficiencies are corrected. The system has completed the following: 1) installed proper vent on well head, 2) sump pump is available to remove any standing water at well head, 3) test results have been submitted for arsenic, annual nitrate, and quarterly coliform, 4) system was provided form templates for operations and maintenance manual, emergency response plan, and coliform sampling plan, to complete and keep on file. ACTION NEEDED: All deficiencies corrected. No further action required. | ||
