| Details: | SUMMARY: Significant Deficiency Follow-up, Phone Conversation & Email DETAILS: After recognizing that the 18 week deadline was approaching, we contacted the ORPD office via phone to speak with the operator. After our phone conversation, we sent an e-mail summarizing the key points of our conversation. Our e-mail said:“Mr. Littrell... We wish to remind you that a deadline to correct an outstanding significant at the well serving Starvation Creek Park wayside is approaching (8/3/16)-- see the attached WSS Report for details. By that date, ORPD is obligated by rule to correct the significant deficiency observed at the electrical conduit on the well head, or, be on an approved corrective action plan/schedule."Our last formal contact on this matter occurred on 4/15/16-- see the attached e-mail. ACTION NEEDED: Please submit written verification that either: • This issue has been resolved prior to 8/3/16-- or • Submit an approvable corrective action plan (detailing proposed work and specific timelines) prior to 8/3/16 for consideration.If the deadline passes, Oregon State Rules require that ORPD post a ‘tier 2’ public notice within 30 days.
