Details:	SUMMARY: MPA DETAILS: Per a Groundwater Source Evaluation Request requested by Justin Fimbres and prepared by Shawn Stevenson a Microscopic Particulate Analysis was required due to the proximity of the well to a surface water. (< 500’)Civil West prepared the MPA sample between 2:00 pm on March 22nd and 10:00 am on March 23rd. The sample was delivered to the Grants Pass Water Laboratory on March 23. Due to a malfunction with the testing equipment at the lab, the sample was not able to be tested within the timeframe required. The GPWL resampled the water between April 10th and 11th and analyzed the sample at the lab. The results are attached, but indicate a Numerical Risk Factor Total of 1, which has a “Low” Relative Risk ACTION NEEDED: A 2nd sample is required no sooner than 30 days from the initial sample. Civil West will collect the sample May 10th.
