  Comments:
The deficiencies noted are as follows: Finished Water Storage Deficiencies 1. Assure flap valves are in place on reservoir drains and overflows. Management and Operation Deficiencies 2. A written protocol for operators is required such that under-certified operators make operational decisions according to the protocol established by the operator in Direct Responsible Charge. Please see the enclosed information. { OAR 333-061-0225 (5) } 3. Submit 2007's Consumer Confidence Report to this office.
