The deficiencies noted are as follows: 1. Often no chlorine in system - CT values not met at all times. 2. Chemicals not NSF-approved. 3. Storage tank hatches not locked. 4. No narrative in written Coliform Sampling Plan 5. Emergency Response Plan out of date 6. No written standard operating protocols for other operators.
