  Comments:
The deficiencies are as follows:
1. Asbestos monitoring. Most recent asbestos data on file is 1993. Please collect one sample downstream from the A/C piping during the warmest month.
2. No written operating protocol for other operators.
3. No cross connection backflow prevention annual summary report.
