1. A screen or flap valve must be installed on the outlet of the tank overflow/drain to prevent blocking or entry of contamination. The connection with the backflow pond must be considered before installing the flap valve.

Treatment Deficiencies
2. Free chlorine residual is not measured twice per week in distribution system.

Management & Operations Deficiencies
3. No consumer confidence reports have been received at this office.
4. The last cross connection annual summary report received was in 2000. While no cross connections exist at Rhododendron, an ASR is still required.
