| Comments: The significant deficiencies and rule violations noted are as follows (see http://www.oregon.gov/DHS/ph/dwp/docs/pwsrules/pwsrules.pdf for a copy of the drinking water rules): 1. Monitoring not current, missed 4th quarter 2008 and 1st quarter 2009 (see OAR 333-061-0025(1)). 2. No Coliform Sampling Plan (see attached Coliform Sampling Plan Form and OAR 333-061-0036(6)(b)(F)). In addition, I have the following comments and recommendations: 1. Add a backflow prevention device between the well and irrigation system.
