The significant deficiencies and rule violations noted are as follows:

Source Deficiencies
1. Ensure vent in the wellhead at well #6 is properly screened with a down-turned elbow.

Management & Operations Deficiencies
2. The Emergency Response Plan (ERP) was not readily accessible for DWP review during the survey.

Recommendations
* Screen vents at well # 3, and assure electrical conduit at well #8 is properly sealed. The screen was partially lifted off at well #3.
* Identify repeat and source sample sites for each routine location in your coliform sampling plan. This will best prevent us of invalid sample sites when repeat sampling.
* Develop a routine line flushing schedule, and include it in the Operation & Maintenance Manual.
* Please submit DRC form to the DWP to designate Jamie Clark as the new DRC for distribution.
* New monitoring reductions:
- Inorganic compounds monitoring at entry points A, B, & C is once every nine years.
- Disinfection by-products monitoring is annual during the summer.
- Monitoring for Volatile Organics Compounds has been reduced to annual, and
- Synthetic Organic Compounds is once every three years at Entry Point D.
This site visit has not been entered into the deficiency database.
