  Comments:
no screened vent on well casing. no treated water sample tap. - add if going to chlorinate crack in concrete reservoir piping configuration needs investigation to verify there is a drain for the reservoir and it has a flap valve on it and can be taken offline or bypassed. facility map needed for water system. routine flushing of dist is needed. no cross connection annual summary report since 2002 lead and copper needed by end of sept 2009. missing 2quarters of rads need coliform sampling plan need ops and maint manual need emergency response plan need certified operator need written porotocols need consumer confidence reports for 2007 & 2008 rg 12/16/09
