Comments: The following are comments and recommendations based on the survey site visit. We recommend that you address these items as soon as possible. Well: 1. Seal the power cord hole visible in the well sanitary seal right away. It is important that the sanitary seal be watertight. 2. Consider eventually replacing the sanitary seal as it is old and rusty indicating the potential for contamination to enter the source. Monitoring: 3. Continue quarterly coliform and annual Nitrate monitoring. 4. Take a one-time Arsenic sample. 5. The School did a good job responding to the coliform positive in January 2009, by disinfecting the well, providing public notice and taking the appropriate follow-up samples in February. 6. Prepare a coliform sampling plan for the system. See attached template which includes a sampling description, and space for identifying three samples to rotate through every three quarters. There are instructions on where to take the repeat and source samples. Three repeat samples and a sample at the well should be taken after any routine coliform positive. Non-swivel taps should be used, and any attachments should be removed from outside hose bibs. Do not use frost free taps for sampling. Operation & Maintenance: 7. Develop an Emergency Response Plan for the water system. This plan includes important contact information and various actions required to respond to potential emergency scenarios you may encounter. Please see a template plan for small systems at: http://www.oregon.gov/DHS/ph/dwp/docs/security/SmallSystemVA-ERP.doc. Attached is a summary of key required elements of the plan.
