| Comments: The significant deficiencies and rule violations noted are as follows: Distribution System: 1. All cross-connection control devices must be tested at least annually, as required by Oregon Administrative Rules (OAR) 333-061-0070 (5), to ensure that the potable water supply is protected from any identified potential backflow hazards in the distribution system. - Corrected 6/28/10 Treatment/Disinfection: 2. Chlorine residual monitoring in the distribution system must be performed twice a week, pursuant to OAR 333-061-0036 (9)(a). The chlorine residual can be measured at a representative location that is convenient for the District. Records should be kept on-site for future review during site visits. - Corrected 6/28/10.
