Comments:
Significant Deficiencies are as follows:
- The collar securing the cap to the casing on the Westbound Well is broken and needs repair.
- The vent screen for the Eastbound Well should be replaced and properly secured to prevent animals and insects from entering the well column.
- Please verify that the chlorine solution tanks are certified to meet National Sanitation Foundation Standard 61 for components in contact with drinking water.
- The Westbound Well drilled in 1989 will need to go through plan review. I have enclosed the plan review requirements and fee schedule for reference. Please contact Marsha Fox at 971-673-0408 for more information on the plan review process.
- The filtration system for iron removal installed on the westbound side will need to go through plan review.
- The State has no record of an initial arsenic sample for the Westbound Well. Please collect a sample as soon as possible.
- You are required to have a written operations and maintenance manual. This manual would include start-up and shutdown procedures, in addition to routine inspections and maintenance tasks for the wells, pumps, equipment for treatment, and distribution piping.
