| Comments: The significant deficiencies and rule violations noted are as follows: 1. The top of the well casing is not water tight. The hole for the submersible pump wiring is not sealed and needs to be plugged with sealant (see OAR 333-061-0050(2)). 2. The water system needs to complete an operations and maintenance manual (see Attachment 3 and OAR 333-061-0065). 3. Silver Lake Motel and Store shall collect their 2011 nitrate sample during the fourth quarter (see Attachment 2 – Monitoring Schedules).
