  Comments:
The significant deficiencies and rule violations noted are as follows: 1. No coliform sampling plan (see OAR 333-061-0036(6)(b)(G)). Please use the attached Coliform Sampling Plan template when drafting a new site plan. 2. Casing not watertight (vent and wire conduit hole open, see OAR 333-061-0076). 3. No raw water sample tap (see OAR 333-061-007). 4. Previous 12 months of coliform monitoring is not up to date. The 2nd quarter 2010 coliform sample was not submitted to the State (see OAR 333-061-0025(1)). The following are comments and recommendations: 1. Remember to collect the required yearly nitrate and quarterly coliform samples (2nd, 3rd and 4th quarters). 2. If a routine quarterly sample tests positive for coliform, please remember to collect 4 repeat samples (one at the positive location, one upstream of the positive sample, one downstream of the positive sample and one directly from the well).
