Comments & Recommendations 1. A copy of Malhuer County's ERP booklet for small water systems was given to the operator at time of inspection. Please fill out the booklet and send me the ERP/VA Proof of Completion(last page) by January 25th, 2011. 2.Please complete the plan review as required. The last item that must be submitted are the nitrate and arsenic test results.
