1. No flap, valve, screen or equivalent on drain.
2. No screened vent.

Monitoring Deficiencies
1. Sampling not current.

The recommendations noted are as follows:

1. A summary of monitoring requirements was discussed. Coliform samples due quarterly, next Nitrate sample due January 2011, and
raw source water sample will need to be taken annually.
2. Please note that the critical deficiencies noted on your survey for treatment are not required to be corrected at this time since you are
choosing to treat your system. If in the future it is determined that treatment is required, you will be asked to acquire an NSF approved
UV unit and proceed through the plan review process and the bypass to the UV light that is currently in the system will need to be omitted.
