The deficiencies noted are as follows:

Management & Operations Deficiencies
1. No Operation and Maintenance Manual.

Monitoring Deficiencies
1. No coliform sampling plan.

Treatment Deficiencies
1. Non NSF approved tank used for holding chlorine.

The comments & recommendations noted are as follows:

1. 2 rounds (6 months) of lead and copper sampling (20 samples each) will be required after the water filtration system comes online.
2. There are no annual source test results for coliform. Systems that use residual maintenance must test each source annually.
3. Water systems that received a VSS Waiver or 40/30 Certification are exempt from IDSE requirements; however, these systems are
still required to complete a Stage 2 Compliance Monitoring Plan (CMP).
4. Notify the State DWP when the filtration plant project is complete. If changes are made a set of as-built plans must be submitted.
5. The DWP has established criteria for determining whether a system should be considered to have "Outstanding Performance".
Systems that are designated outstanding performers may have their water system survey frequency reduced from every 3 years
to 5 years. Although your water system did not meet the established criteria, please review the enclosed handout to see what steps
you can take in the future towards receiving this designation.
6. A summary of your monitoring requirements can be found on page 8. Please maintain a copy of this page and refer to it for future monitoring scheduling.
