1. Sanitary seal and casing not watertight.
2. No raw water sample tap.

The recommendations and comments noted are as follows:

1. Please replace the sanitary seal on the well to make it water tight. The new seal should also have a proper screen.
2. Please provide a raw water sample tap. This would be located on the system prior to any treatment.
3. Please make sure that you are submitting quarterly coliform sampling as required.
4. The nitrate MCL violations need to be addressed. You must maintain the public notice until this issue is addressed.
5. An ERP is being sent along with this survey. Please fill out the packet and return the page that is titled
"Emergency Reponse Plan/VA Proof of Completion". Please do not send the entire packet. This is for your records only.
