The deficiencies noted are as follows:

1. Major modifications made without prior plan review approval.

1. 4th quarter coliform test will need to be completed before the end of the year.
2. Be sure all wells with a vent have a secure screen.
3. Building for well #8 appears to have a leak in the room and is in need of repair. Floor drain in well #5 building is plugged and in need of repair.
