The deficiencies noted are as follows:

Management & Operations Deficiencies
1. No Operation and Maintenance Manual.
2. No Emergency Response Plan.

Operator Certification Violations
1. No DRC certified at the appropriate level.

The recommendations and comments noted are as follows :

1. In 2004 system had an arsenic level of 0.015 mg/L. At the time, the MCL was 0.05 mg/L. In 2006 the EPA reduced the MCL to 0.01 mg/L.
When sampling this year, if the Arsenic levels (and a confirmation sample) are found to exceed the MCL, treatment may be required.

2. At the time of the survey, it was noted that a concrete slab did not exist around the wellhead. Current construction standards require a
concrete slab at least 3 inches thick which extends around the well casing a minimum of 12 inches in all direction. I would recommend a slab be
poured to help prevent well contamination. Additionally, there appeared to be some rodent activity within the loose insulation used in the well house.
I would recommend replacing the loose insulation with harder foam insulation or other alternative.
