1. Turbidity not measured in correct location.
2. Turbidity standards not met.
3. Turbidity profiles not conducted at least quarterly.
4. Turbidimeters not calibrated according to manufacturers specifications.
5. Tracer study or alternative not conducted.
6. No call-out alarm or auto shut-off for high turbidity.
7. Chlorine residuals are not recorded as required.
8. CT values not being calculated correctly.
9. DPD or EPA approved method of measuring chlorine residual not used.
10. Free chlorine residual not maintained.
11. Minimum CT requirement not met at all times.
12. Contact chamber does not have effluent flow meter.
13. pH, temperature, and chlorine residual not measured daily at first user.
