1. Filter effluent turbidity limits not met
2. Incorrect location for compliance turbidity monitoring.
3. Turbidity profiles not conducted at least quarterly following backwash,
4. Turbidimeters not calibrated per manufacturer’s recommendations or at least quarterly.
5. Peak hourly demand flow not measured as required.
6. Chlorine residuals are not recorded as required.
7. No Operation and Maintenance Manual.
8. No written operating protocols for under-certified operators.
