  Comments:
The significant deficiencies and rule violations noted are as follows: 1. A hole was observed in the side of the casing. A metal plate shall be welded over the opening to prevent animals and debris from contaminating the well (see OAR 333-061-0076). 2. No raw water sample tap (see OAR 333-061-0076). 3. The previous 12 months of coliform monitoring are not up-to-date. Water system needs to start collecting quarterly coliform samples and submitting the results to: DMCE 800 NE Oregon Street, Ste 640 Portland, OR 97232 4. Annual nitrate and the one time arsenic samples are not up-to-date and shall be collected by the end of 2011. The results from the monitoring will need to be submitted to the address listed above (see attached Transient Non-Community & State Regulated Water System Routine Monitoring fact sheet). 5. Water system needs to complete a Coliform Sampling Plan (see attached template). 6. Water system needs to complete an Emergency Response Plan (see attached fact sheet).
