  Comments:
The significant deficiencies and rule violations noted are as follows and should be corrected: 1. All backflow prevention devices need to be tested annually (see OAR 333-061-0070). 2. Complete the operating protocols for other operators (see Attachment 4 - Guidance Documents, Written Protocol for Operators, fact sheet). 3. Annual assessment and triggered monitoring for all the springs is conducted at a combined sample location, which is located at the Springline Chlorination Building. In order to continue monitoring at the combined sample location, the water system must complete a Combined Groundwater Source Coliform Sampling Request form (see Attachment 4). In addition to the deficiencies listed above and in the survey form, the City of Lakeview should address the following recommendations: 1. Recommend diverting surface water from the spring areas. 2. Recommend re-grouting concrete joint on the pressure relief box located prior to town. 3. Recommend updating the Coliform Sampling Plan to include triggered source water monitoring sites (see Attachment 4 - Guidance Documents, Coliform Sampling Plan fact sheets).
