| Comments: The survey of the WTP was conducted on 3/7/12 with Scott Curry (OHA-DWP) present; the reaminder of the survey (storage and distribution portion) was conducted on 4/4/12 without Scott. THERE WERE NO SIGNIFICANT DEFICIENCIES. M&R violations within the last 3 years prevented them from qualifying for Outstanding Performer status. RECOMMENDATIONS: Treatment- Adjust coagulant dose using jar tests when the raw water turbidity fluctuates. Verify on-line chlorimeters weekly. Finished Water Storage - Continue to seal leaks on the exterior walls. Establish an inspection schedule for the reservoir interior, and use these findings to develop an appropriate interval for cleaning. Distribution - Establish a schedule for routine turning of distribution valves. Monitoring and Reporting - Submit raw alkalinity quarterly along with the TOC results. If the seasonal source (well) is used, must submit one assessment monitoring sample per year. Management & Operations - Continue to organize the operator protocols so that it is easy for any operator to find those that are current.
