SIGNIFICANT DEFICIENCIES: SOURCE - No treated sample tap at Well 4. TREATMENT - Chlorine residuals not measured and recorded at least twice weekly in the distribution system. RECOMMENDATIONS: CHEMICAL SAMPLING LOCATIONS - Entry point samples must be taken after (chlorine gas) treatment and storage. SECURITY: Add a gate to road access to well heads and reservoir. STORAGE TANK: Develop an inspection schedule (visual, once a year) and cleaning schedule based on the inspection results. MONITORING: The monitoring schedule for DBPs had been interrupted in our database. Hence, OIT was overdue for these samples, and we recommended they take them right away, in summer 2012.
