Construction waiver obtained for diesel storage tank at well site. Treated sample tap installed at well site. Using correct peak hourly flow rate on monthly report. Chlorine residual is measured and recorded at least twice a week in distribution. Emergency Response Plan completed. Coliform Sampling Plan revised. Written protocols available. Also reviewed revisions to O&M Manual during visit. *All significant deficiencies corrected and recommendations adopted from 5/11/11 survey and 8/8/12 treatment plant inspections.
