The deficiencies noted are as follows:

TreatmentDeficiencies
1.No individual pressure gauges before and after each compliance pre and final filter, as required by OAR 333-061-0076 (4)(a)(F). Monitor the pressure differentials measured across each filter, and continue to track the maximum differentials across the set of pre and final filters, in the monthly reporting form submitted to the DWP.
2.No individual flow restrictors after each parallel pre and final filter train. Individual flow restrictors are required after each final filter.
3.Disinfection CT’s not calculated daily as required by OAR 333-061-0036 (5)(b)(B). Be sure to completely calculate CT-achieved and CT-required every day the plant is in operation to ensure adequate disinfection is occurring.

Monitoring and Reporting Deficiencies
4.Raw water Total Organic Carbon (TOC) not monitored and reported as required by OAR 333-061-0036 (4)(e)(C). Systems on reduced Disinfection Byproduct (DBP) monitoring are required to sample the DBP precursor, TOC, prior to treatment on a quarterly basis to maintain the DBP monitoring reduction for total trihalomethanes and haloacetic acids from quarterly to annually. It was noted that the DWP received the July 15, 2012 raw water TOC sample result on August 7, 2012, after the survey.

Management & Operations Deficiencies
5. No written operator decision-making protocol for operators not certified at the level of the water system, as required by OAR 333-061-0225 (2). This protocol should list what procedures operators can do and under what circumstances the operator in Direct Responsible Charge (DRC) should be consulted. This procedure should be signed by all operators and kept on site.
