The deficiencies noted are as follows:

Treatment Deficiencies
1. Develop Standard Operating Procedures for all operators.
2. Identify whether you need to install the hold-down devices required by the challenge test. The required devices are 5V-2787, while the installed hold-down devices are 2150 SE SP BHD.
3. Develop an alarm system or man the plant during all hours of operation.

Monitoring Deficiencies
4. Develop Coliform Sampling Plan (see template enclosed & online).
5. Combined Radium and Combined Uranium monitoring overdue.

Management & Operations Deficiencies
6. Develop and update an Operations and Maintenance Manual and make it accessible.
7. The 2005 Consumer Confidence Report was due 1 July 2006.
