Defiencies noted are as follows: 1. No raw water sample tap. Cl2 injected directly on top of well in discharge piping. 2. No contact time. 3. No NSF-approved label on Cl2 jugs. 4. No written coliform sampling plan. 5. No O&M Manual. 6. No written standard operating protocols for other operators. 7. No certification letter for 2004 and 2005 CCRs. Recommendations: Garbage bins should not be used for Cl2 dosing tanks.
