Finished Water Storage Deficiencies
1. Assure a flap valve or equivalent is in place on reservoir overflow to prevent blocking or entry of contamination.

Distribution System Deficiencies
2. A cross-connection ordinance or other enabling authority must be developed { OAR 333-061-0070 }
3. Cross-connection records must be kept current. All backflow prevention devices must be tested annually.

Monitoring Deficiencies
