| Comments: Corrective Action Plan Due June 13, 2008 for the following deficiencies: 1) No watertight access hatch/entry for the Pumper Well Spring (OAR 333-061-0050(2)(b)(A)(iii)). Springs considered to be groundwater sources should be constructed so that surface water is excluded. The crack shown in the picture below may let surface water enter the spring. 2) Settled water turbidity not measured daily (OAR 333-061-0076(3)(a)(D)). In order to maintain the 2.5 log giardia removal credit, it must be demonstrated that the sedimentation process is not the limiting performance factor for operating during high filtration rates. 3) Turbidity profile not conducted at least quarterly (OAR 333-061-0076(3)(a)(D)) 4) Nitrate results for 2007 were not reported for entry point A (EP-A) for the South Yamhill River source. (Corrected results for EP-A in 2007 were received 05/13/08 - Lab Sample Number 20070808-026. - results were submitted for data entry and violation resolution - eh 05/15/08.)
