| Comments: Following deficiencies were noted: 1) Casings at several wells (#4,5) must be made watertight. 2) Nitrate results were not reported for 2007 for either entry point. Sampling must be conducted as soon as possible and results submitted. 3) A written coliform plan must be developed. 4) An operations/maintenance manual must be written. 5) Written protocols for operators must be developed. 6) An emergency response plan must be developed.
